Home from Hospital

  • VO99436

Description

You will offer a friendly, confidential service to people as they return home from a hospital stay or to someone in the community as prevention of admission. This involves one-to-one, short-term, practical support, so that patients can return home with confidence when no other support is available. You could help with their shopping, collecting their prescriptions, one-to-one walks, transport to medical/health or social appointments or just being there when someone starts to do something for themselves.

You'll need a friendly personality and be able to spot areas of concern. Good communication and listening skills are a must, as well as a non-judgemental attitude towards someone's circumstances and be able to keep things confidential. Sometimes, you'll be lone working building relationships with the people you visit, something which can be rewarding for you and the people you support. This role requires a PVG which will be met by RVS.
