Description

An exciting opportunity to make a real difference in your community.

As a Community Health Champion you will help the elderly in your community, once discharged from hospital, to regain their independence, confidence and participate in community life.

This can range from making sure that their house is warm and there are basic foods in for their return, to encouraging and motivating them in their recovery by volunteering alongside the START team, offering social and practical support, collect prescriptions and basic shopping, pop in for a chat, telephone support, and engage in local activities.

Organisation aims and objectives

Working directly within the community in partnership with statutory health and social care services to provide additional support through volunteering.
